TRT Side Effects and Monitoring: What Follow-Up Should Cover

TRT monitoring should assess whether treatment is helping and whether it remains appropriate. Testosterone levels are only one part of that review. Blood counts, blood pressure, symptoms, fertility plans, prostate considerations, correct use, and new medical history can all change the decision. This guide explains the main follow-up topics for men receiving prescribed testosterone for hypogonadism and how to prepare for a useful review without turning isolated test results into a self-directed dosing plan.

Establish the baseline before the first prescription

Treatment should follow a supported diagnosis: compatible symptoms or signs, consistently low testosterone, and evaluation of the cause. Baseline safety assessment then asks different questions, including whether a high hematocrit, untreated severe sleep apnea, recent cardiovascular event, or other condition affects suitability. The Endocrine Society guideline describes situations in which it recommends against starting therapy.

Bring prior blood counts, relevant specialist records, medicines, and any history of testosterone or anabolic steroid use. Fertility goals must also be discussed before treatment. A baseline makes later changes easier to interpret; without it, a high result during therapy may be harder to distinguish from a preexisting condition that already needed evaluation.

Testosterone measurements need a timing plan

The meaning of a follow-up testosterone concentration depends on the formulation, dosing schedule, and time since the last dose. Injection and gel monitoring cannot be reduced to one universal blood-draw instruction. The clinician should specify the timing and explain what dose information to provide with the sample.

The AUA recommends checking testosterone after an appropriate interval when therapy begins and measuring levels every six to twelve months during ongoing treatment, with the schedule tailored to the situation. The AUA guideline discusses formulation-specific considerations. Do not skip, add, or shift doses independently to produce a preferred laboratory result. The aim is to understand usual exposure and whether the prescription is achieving an appropriate clinical response.

Hematocrit can rise during treatment

Testosterone can stimulate red blood cell production. Hematocrit measures the percentage of blood volume occupied by red cells, while hemoglobin measures a related but different feature of those cells. A complete blood count helps monitor both. An excessive rise may require a change in the treatment plan and evaluation of contributing factors.

The AUA advises considering withholding treatment when baseline hematocrit exceeds 50% until the cause is clarified and recommends intervention at 54% or higher during therapy. Blood pressure remains important after the FDA update

In February 2025, the FDA announced removal of boxed-warning language about increased major adverse cardiovascular outcomes after reviewing TRAVERSE. It also required new or expanded warnings about increased blood pressure based on ambulatory monitoring studies. Both parts of the update matter. The FDA announcement does not establish that cardiovascular monitoring is unnecessary.

Know your baseline pressure and follow the clinician’s plan for reassessment. If home monitoring is advised, use an appropriate validated upper-arm device and consistent technique. Bring the readings and device questions to follow-up. Do not stop blood-pressure medicine or alter testosterone based on one isolated reading. Persistent elevations need review in the context of overall cardiovascular health and the current prescription.

Prostate assessment should fit age and risk

Prostate monitoring is not identical for every man. Age, family history, prior findings, symptoms, and preferences affect the discussion about prostate-specific antigen, or PSA, and examination. A PSA change does not automatically mean cancer, but it may require further assessment rather than being dismissed as a predictable effect of TRT.

The Endocrine Society recommends shared decision-making about prostate monitoring in relevant age and risk groups and urologic evaluation for specified concerning findings. Its guideline provides the framework. Tell the clinician about difficulty urinating, a weak stream, blood in urine, prior prostate procedures, or an abnormal PSA history. A hormone level cannot replace evaluation of a new urinary or prostate concern.

Sleep and breathing symptoms deserve attention

Untreated severe obstructive sleep apnea is among the conditions the Endocrine Society identifies as a reason not to start testosterone therapy. During follow-up, report loud snoring, witnessed breathing pauses, gasping, or substantial daytime sleepiness. These symptoms may indicate a problem that needs its own evaluation.

Sleep-disordered breathing can also be relevant when hematocrit is elevated. The clinician may investigate contributing hypoxia rather than assuming testosterone is the only factor. Do not rely on improved energy as proof that a sleep disorder is controlled. If sleepiness makes driving unsafe, address it promptly. Skin, fluid, and local treatment effects can affect tolerability

Acne, oily skin, swelling, breast symptoms, application-site irritation, or injection-site discomfort may occur with testosterone treatment or warrant evaluation during it. Record the timing and severity, the exact product, and any recent changes. A persistent symptom should be reviewed rather than managed by layering additional medicines onto the prescription without guidance.

For injections, spreading redness, fever, or severe site pain needs prompt assessment. Fertility plans can change during therapy

Even if children were not part of the original plan, bring up any new interest in future fertility. External testosterone can suppress sperm production, and a normal on-treatment blood testosterone level does not measure reproductive capacity. The AUA/ASRM guideline advises against exogenous testosterone in men interested in current or future fertility. The guideline explains this concern.

Do not assume a shorter course, lower dose, or gel formulation eliminates suppression. Recovery after stopping can take months or, rarely, years and cannot be guaranteed on an individual schedule. Know which symptoms need urgent care

Chest pain, sudden shortness of breath, stroke-like symptoms, or a painful swollen leg require urgent medical assessment. Severe allergic symptoms or other acute illness also should not be managed by adjusting testosterone at home. Tell the treating team what product you use and when the last dose was taken.

Less urgent symptoms still deserve a clear contact route. Ask how the practice handles a rising blood pressure trend, persistent headaches, swelling, skin reactions, or unexpected changes in mood or urinary function. Benefit is part of the continuation decision

Monitoring is incomplete if it checks only laboratory values. Compare the original symptom goal with the current experience. If testosterone normalizes but symptoms or signs do not improve, the AUA recommends discussing cessation after three to six months. This should occur with the prescriber, including consideration of the diagnosis, correct use, and other causes of the symptoms.

A higher number can show drug exposure without demonstrating meaningful benefit. Conversely, a perceived benefit does not justify ignoring adverse findings. Do not turn follow-up into an accessory-drug routine

An elevated hematocrit, breast symptom, or unexpected hormone result does not automatically establish a need for blood donation, an aromatase inhibitor, aspirin, or another hormone. Those interventions have their own indications and risks. The clinician should first assess the actual finding, formulation, exposure, contributing conditions, and evidence for the proposed response.

Bring every medicine and supplement to review, including products obtained outside the prescribing practice. Avoid self-designed “optimization” stacks or repeated dose changes driven by online targets. If a proposed additional treatment is unclear, ask what diagnosis it addresses, what benefit is expected, and how its own safety will be monitored. More prescriptions do not necessarily mean a more complete plan.

Prepare a short follow-up record

Include the product and schedule, last dose relative to testing, missed doses, original goals, improvements, side effects, blood-pressure readings if requested, and any new diagnoses or fertility plans. Bring the actual laboratory report with units and reference ranges. A concise record makes the visit easier to interpret than a collection of isolated numbers.
