Before Starting TRT: A Practical Guide to Evaluation and Treatment Planning

A first TRT consultation should establish what problem is being treated, whether testosterone deficiency is confirmed, and how treatment would fit your health and future plans. You do not need to arrive with a preferred dose or a list of add-on medicines. Bring the symptoms, prior tests, medical history, and questions that affect the decision. This guide helps organize a testosterone replacement discussion for men with possible hypogonadism, from diagnosis and fertility to product instructions and follow-up.

Describe the concern in everyday terms

Identify the main change that led you to seek care. Reduced sexual desire, fewer spontaneous erections, persistent fatigue, or declining function may raise different questions. Note when the change began, whether it is consistent, and how it affects work, relationships, sleep, or activity. Include what you have already tried.

Symptoms alone do not establish testosterone deficiency, and a low result does not prove that testosterone explains every concern. The Endocrine Society guideline recommends a diagnosis based on compatible symptoms or signs and consistently low concentrations. A clear description keeps the consultation focused on the original problem rather than treating a laboratory number as the only outcome that matters.

Bring complete laboratory reports

Include the collection date and time, units, reference ranges, and whether testing occurred during illness or while using hormones. The AUA recommends two early-morning total testosterone measurements on separate occasions, while the Endocrine Society recommends repeat morning fasting testing to confirm a low result. Ask what could be causing the low level

Testosterone deficiency can involve the testes or the signaling system in the hypothalamus and pituitary. Measurements such as LH and FSH help distinguish those patterns. Additional testing depends on the history and findings rather than a universal panel for every patient.

Ask whether the clinician has identified a likely cause and what further evaluation would change treatment. Medicines, serious illness, obesity, sleep problems, and prior hormone use may be relevant. The Endocrine Society guideline recommends evaluating the cause of androgen deficiency. Starting treatment before that process is complete can change the measurements and complicate later assessment. State fertility plans before discussing a formulation

Say whether children are desired now, later, or possibly. External testosterone can suppress sperm production, and a normal blood testosterone level during treatment does not establish fertility. Ask whether semen testing, reproductive specialist consultation, or discussion of alternatives should happen first. Do not assume a gel, low dose, or add-on medicine guarantees preservation of sperm production. Recovery after stopping can be prolonged and cannot be promised on a personal timetable. Bring the history that affects safety

Include blood-pressure problems, heart disease, prior heart attack or stroke, blood clots, sleep apnea, lung disease, kidney conditions, urinary symptoms, prostate findings, and cancer history. Note the dates of major events and whether the conditions are currently controlled. Bring a recent blood count if available.

The Endocrine Society identifies several circumstances in which it recommends against starting testosterone, including elevated hematocrit, untreated severe sleep apnea, uncontrolled heart failure, recent heart attack or stroke, and specified prostate or cancer concerns. The clinician should explain how your actual history affects the choice. A normal testosterone follow-up target cannot compensate for an unresolved safety issue that needs assessment before treatment.

Disclose all medicines and prior hormone use

List prescription medicines, supplements, injections, gels, anabolic steroids, and products marketed as testosterone boosters. Include use from other practices or online sources and the approximate start and stop dates. Opioids and glucocorticoids can be relevant to hormone status, while other medicines may affect symptoms or interact with treatment.

This history helps interpret both diagnosis and safety. Do not assume a product is irrelevant because it was called natural or because it was used months earlier. Bring labels or photographs if names are unclear. The clinician needs an accurate exposure history to avoid accidental duplication and to understand why hormone measurements or symptoms may not reflect an untreated baseline.

Ask what benefit is realistic

Agree on a specific outcome and how it will be measured. Sexual symptoms, energy, mood, muscle, and physical function are separate questions. Research does not support promising that all will improve together or on a fixed schedule. Ask what should happen if the testosterone level normalizes but the original symptom does not improve. The AUA recommends discussing cessation after three to six months in that situation. Compare exact products and routines

If TRT is appropriate, ask which formulation is proposed and why it fits your history and daily life. Injections and gels differ in handling, exposure patterns, testing timing, local adverse effects, and practical demands. Testosterone cypionate is one injectable formulation; the cited Depo-Testosterone label specifies intramuscular use. Other routes require explicit prescribing context. For gels, ask about application areas, drying, skin contact, handwashing, and prevention of transfer to partners or children. For injections, ask about training, supplies, and sharps disposal. Clarify approved use and off-label decisions

The FDA’s 2025 testosterone labeling update retained the limitation concerning age-related hypogonadism. It removed certain boxed-warning language about major cardiovascular outcomes after TRAVERSE and added or expanded blood-pressure warnings. The FDA announcement does not make testosterone a general anti-aging prescription.

If the proposed use, route, or compounded formulation differs from an approved product’s labeling, ask what that means and why it is being recommended. Off-label prescribing should have a clear clinical rationale and understandable instructions. If compounding is proposed, ask what need the approved options do not meet; compounded drugs do not undergo the same FDA premarket review. Set the monitoring schedule before starting

Ask which baseline tests are required, when follow-up testosterone should be measured, and how timing relates to the last dose. Clarify the plan for hematocrit, blood pressure, symptom response, and prostate assessment appropriate to your age and risk. Monitoring should include both benefit and adverse findings.

The AUA recommends ongoing testosterone measurements every six to twelve months, with earlier assessment during initiation or adjustment. The exact schedule depends on the clinical situation. Obtain instructions for ordinary problems

Before using the prescription, know what to do after a missed dose, how to store it, and what happens if the pharmacy supplies a different concentration or product. Do not calculate a conversion from an old label or overlap formulations without guidance. For home injections, confirm the written instructions and training before administering anything.

Ask how to contact the practice about skin irritation, injection-site problems, swelling, persistent headaches, urinary changes, or elevated home blood-pressure readings. Also clarify urgent symptoms such as chest pain, sudden shortness of breath, or stroke-like symptoms, which require urgent medical care. A clear plan should cover both routine use and the situations that cannot wait for the next scheduled appointment.

Leave with an understandable next step

The outcome may be repeat testing, evaluation of another condition, specialist referral, a prescription, or a decision against TRT. Each can be useful if it addresses the original concern and explains what happens next. Ask the clinician to summarize the diagnosis, likely benefit, main risks, uncertainties, and review plan in plain language.

Save the instructions, baseline symptom record, and laboratory reports for follow-up. Revisit fertility, medical history, and goals when circumstances change.
