Testosterone replacement therapy, or TRT, treats appropriately diagnosed testosterone deficiency. The decision requires symptoms or signs together with consistently low testosterone levels and an evaluation of the cause. Fatigue, aging, or one low result is not enough by itself. This guide explains the diagnostic process, realistic benefits, treatment choices, fertility implications, and follow-up. It focuses on TRT for men with hypogonadism; other uses of testosterone have different clinical frameworks.
Diagnosis combines symptoms with reliable measurements
Low sexual desire, reduced spontaneous erections, unexplained anemia, or changes in physical function may raise the question of testosterone deficiency. Many symptoms are nonspecific, however. Poor sleep, depression, medication effects, obesity, and other illnesses can produce overlapping concerns. A symptom checklist cannot establish the diagnosis on its own.
The Endocrine Society recommends diagnosing hypogonadism only when compatible symptoms or signs occur with unequivocally and consistently low testosterone concentrations. It recommends confirming the result with a repeat morning fasting measurement. Its clinical practice guideline explains this process. One cutoff does not replace clinical interpretation
The American Urological Association uses a total testosterone level below 300 ng/dL as a reasonable cutoff supporting the diagnosis, alongside symptoms and two early-morning measurements on separate occasions. Reference ranges and assay methods differ, and selected situations may require an appropriately measured or calculated free testosterone assessment. The AUA guideline provides the framework.
Do not treat a single afternoon value or a result obtained during an acute illness as a complete answer. The clinician should review the circumstances, medicines, and prior results. The objective is to identify a clinically meaningful deficiency, not to move every person toward the highest number on a laboratory range or a target promoted online.

Finding the cause is part of treatment planning
Low testosterone can reflect a problem in the testes or in the brain’s signaling system that directs hormone production. Measurements such as luteinizing hormone and follicle-stimulating hormone can help distinguish those patterns. Additional evaluation depends on the findings and history, including medication use, illness, fertility concerns, and signs suggesting a pituitary disorder.
This matters because treatment of an underlying condition may be relevant and because not every low level calls for the same approach. The Endocrine Society recommends further evaluation to identify the cause of androgen deficiency. Starting testosterone before the diagnostic process is complete can also change hormone measurements and make later interpretation more difficult. Bring previous results and disclose any current or prior hormone use.
Benefits should match the evidence and your goals
TRT may improve selected symptoms in appropriately diagnosed men, particularly sexual symptoms, and can affect anemia and body composition in some settings. It does not guarantee restored energy, weight loss, stronger athletic performance, or improved mood for every patient. A defined baseline makes it easier to judge whether a benefit is occurring.
In the Testosterone Trials, 790 men aged 65 or older with low testosterone were assigned to testosterone gel or placebo for one year. Sexual function improved moderately, while the primary vitality and physical-function outcomes were not clearly improved as originally defined. The trial report supports a measured discussion. Formulations differ in use and monitoring
Testosterone is available in several prescription formulations, including injections and topical gels. They differ in absorption, dosing schedules, handling, side effects, and the timing used for follow-up blood tests. A convenient schedule is relevant, but it should be considered alongside the patient’s health history and ability to use the medicine correctly.
Testosterone cypionate is one injectable ester. The cited Depo-Testosterone label specifies intramuscular administration; some clinical use of cypionate by another route is off-label and requires explicit prescribing instructions. It should not be assumed interchangeable with every other injectable testosterone. The current product label provides formulation-specific information. Fertility plans belong at the beginning
External testosterone can suppress the hormonal signals needed for sperm production. A normal blood testosterone concentration during treatment does not establish normal fertility. The AUA/ASRM male infertility guideline advises against prescribing exogenous testosterone to men interested in current or future fertility. The guideline makes this a central treatment-planning issue.
Discuss future family plans even if pregnancy is not an immediate goal. Recovery of sperm production after stopping can take months or, rarely, years, and an individual recovery date cannot be guaranteed. TRT is also not reliable contraception. Baseline safety review looks beyond testosterone
The clinician should review a blood count, medical history, cardiovascular status, sleep symptoms, urinary symptoms, and prostate considerations appropriate to age and risk. Elevated hematocrit, untreated severe obstructive sleep apnea, uncontrolled heart failure, recent heart attack or stroke, thrombophilia, or certain prostate findings can change whether treatment is appropriate.
The Endocrine Society lists circumstances in which it recommends against starting therapy, including near-term fertility plans and specified cancer or prostate concerns. Its guideline should be applied to the individual situation. A reassuring testosterone number does not override an unresolved safety issue. Bring a complete medication list and tell the clinician about prior nonprescribed testosterone, anabolic steroids, or products marketed as hormone boosters.
The current cardiovascular message has two parts
In February 2025, the FDA announced removal of boxed-warning language about increased major adverse cardiovascular outcomes after reviewing TRAVERSE. It also required new or expanded blood-pressure warnings following ambulatory blood-pressure studies. The limitation concerning age-related hypogonadism remained. The FDA’s announcement describes both changes.
TRAVERSE tested testosterone gel in men with confirmed hypogonadism and existing or high cardiovascular risk. It did not establish that every testosterone formulation is risk-free, that TRT prevents heart disease, or that supraphysiologic use is safe. Blood pressure still needs attention. Hematocrit is a separate monitoring priority
Testosterone can increase red blood cell production. Hematocrit measures the proportion of blood volume occupied by red cells, and an excessive rise can require treatment review. It is not the same measurement as testosterone or blood pressure. A blood count helps establish the baseline and monitor changes during therapy.
The AUA advises considering withholding treatment when baseline hematocrit exceeds 50% until the cause is explained and recommends intervention when hematocrit reaches 54% or higher during therapy. The guideline describes clinician-directed management. Do not respond by arranging repeated blood donation, adding aspirin, or changing doses independently. Follow-up should test benefit as well as safety
A follow-up plan should specify when testosterone and other relevant measurements will be checked, how blood-draw timing relates to the formulation, and which symptoms or adverse effects to report. Stable testosterone levels are generally monitored periodically; the AUA recommends measurement every six to twelve months, with earlier assessment during treatment adjustment.
A normalized level without meaningful symptom improvement should prompt a discussion about whether continuing treatment makes sense. The AUA recommends discussing cessation after three to six months in that situation. This is a clinician-guided reassessment, not an instruction to stop a prescription independently. Hormone treatment does not replace general health care
Sleep evaluation, cardiovascular risk management, nutrition, physical activity, and appropriate cancer screening remain important during TRT. Testosterone should not be used solely to improve glucose control in men with type 2 diabetes, according to the Endocrine Society guideline. Nor should changes in lean mass be presented as proof of improved mobility or fewer fractures.
A large TRAVERSE fracture analysis did not show fracture prevention from testosterone and found more clinical fractures in the testosterone group. The original report reinforces why bone density and fracture outcomes are different questions. A person with osteoporosis, falls, or substantial weakness needs a specific assessment rather than relying on TRT as a comprehensive treatment for aging.
Prepare for a decision that can be reassessed
Bring the symptoms you want addressed, prior laboratory results with collection times, medications, fertility plans, and relevant medical history. Ask what diagnosis is being treated, which formulation is proposed, what benefit is realistic, and what would lead to a change or discontinuation. Obtain product-specific instructions and a clear contact plan.
